What does a Fixed Penalty Notice issued by the MetPol under Corona Virus legislation look like?

There is a lot of misinformation and uncertainty about how the police implements their new powers under the more recent Corona Virus Legislation.  This legislation came into force on the 5/11/2020 and it will expire after six months.
